We will provide you with a privacy notice each year as long as you have a relationship with United Bank. This privacy notice applies to information about our current and former customers. If you open another account or obtain a product or service from us, you may receive another copy of this privacy notice. Customer Information we collect and may disclose

In order to deliver the products and services your request easily and efficiently and to help us administer our businesses, we collect the following types of non-public information from various sources:

• Information we receive directly from you such as your name, address, e-mail address and telephone numbers so we can send you information about our products and services and call or write you about your accounts. We also collect social security number and driver's license information or other photo ID information as required by law when you open an account relationship with us. We also collect other information such as marital status, age, income, assets and liabilities and other information from applications and other forms you complete so we can process applications and suggest other products and services that will help you get the most from your relationship with us.
• Loan payment history, account balances, and account usage about transactions you conduct with us, our affiliated organizations and others that would permit us to process your requests quickly and recommend appropriate products or benefits.
• Credit scores or credit history from credit reporting agencies and other information, such as demographic information and insurance claims information we receive from third parties, related to your creditworthiness. We use this information to process loan applications, and monitor and collect on existing accounts.

How this Information Benefits You as a Customer

We use Customer Information to provide you with quality customer service and value for the products and services you obtain from us. Sharing Customer Information can save you time because you may not have to supply our affiliated organizations with the same information.

Accessing Your Information

Our employees have limited access to Customer Information. Employees are trained and permitted to access Customer Information only when it will help you do business with us or help us do business with you. We maintain internal controls that comply with government regulations to safeguard Customer Information.

Restrictions on Sharing Information with Outside Companies

United Bank does not sell Customer Information to outside companies to use for their own marketing purposes. We may provide Customer Information to outside organizations that provide us services (such as printing or mailing) or help us provide you with products or services you request (such as brokerage transactions or underwriting insurance applications) in order to provide you with these products and services. We may also provide Customer Information to companies that perform marketing services for us or to financial institutions with which we have joint marketing agreements. These companies are contractually obligated to keep your Customer Information confidential. We also share Customer Information with others when you specifically request and authorize us to do so. We also share information when it is required or permitted by law, such as when we report to credit bureaus, when we report suspected fraudulent or criminal activity, when we receive subpoenas, or when we are ordered by court summons.

Information Shared Between Our Affiliated Organizations

In order to provide you quality customer service, sharing Customer Information among our affiliated organizations is important. We may share application or credit information about customers with our internal broker(s), insurance representative(s), or other affiliates because they may offer a product or service that may benefit you more than the company with which you are currently dealing.

However, if you do not want us to share application and credit information with another company in our organization, please call us at (877) 516-9417 or (479) 756-8811 and ask for Account Services.

When you call, someone will be able to answer your questions about privacy, and if you choose, you may request that we not share application or credit information within our organization. You should know that if you ask that we not share Customer Information in this way, you may not receive information about our products or services that may benefit or be of interest to you. If you have already called us and requested that we not share this type of Customer Information, there is no need to make this request again.

Please be aware that even if you do ask that we not share application or credit information about you with our affiliated companies, we are permitted by law to share transaction and experience information (i.e. account balances and payment history) within our organization.

If you have a joint account, we will treat a request not to share credit or application information by one account holder as applying only to that account holder unless the customer notifies us that he/she is acting on behalf of the other account holder(s). If your insurance policy names more than one person as the insured parties, we will treat a request not to share this type of information by one insured as applying only to that person unless he/she notifies us that he/she is acting on behalf of the other insured party.

Health Information

We may collect personal health information, like medical reports, for certain types of insurance policies. We do not share personal health information with anyone except as necessary to underwrite the policy, administer claims, or as otherwise requested by you or as required or permitted by law.
